Adaptions to response format from saslauthd

Turns out that saslauth use the same length encoding
on the (single) response string as the multiple
input strings.

In addition fix up warnings returned from coverity

The easiest way to test auth via saslauthd today
is to:

install saslauthd
copy /etc/pam.d/passwd /etc/pam.d/couchbase
export CBAUTH_SOCKPATH=/var/run/saslauthd/mux

edit memcached.json and add:

"sasl_mechanisms" : "PLAIN"

and start memcached manually.

Treat all unknown users as external users

This means that all ns_server needs to do is to update
the privilege map (to be added as part of the RBAC work).

By doing so you can authenticate to the cluster, but you'll be
associated with the no-bucket and disconnected until you
run either select bucket (which you currently can't).

Add support for ldap via saslauthd

ns_server allows for authentication via LDAP by utilizing
saslauthd. This patch adds the same functionality to memcached.

In order for memcached to enable saslauthd, the environment
variable CBAUTH_SOCKPATH must be set to the unix domain socket
to communicate to saslauthd.

The user needs to be defined as external in the cbsasl password
database for it to be looked up in LDAP.
